What Changes in CPT 2027 for Small Practices? The Codes to Remap Before January 1

CPT 2027 brings 453 code changes for small practices on January 1, 2027

CPT 2027 takes effect on January 1, 2027, with 453 changes: 299 new codes, 74 revised codes and 80 deleted codes. For a practice with 1 to 15 providers, the changes that cause denials sit in four places. ABA services get a full rewrite. Global maternity billing is replaced. Speech therapy code 92507 is deleted. Biofeedback moves to time based codes. Office visit E/M codes (99202 to 99215) do not change.

Any claim dated January 1 or later that carries a deleted code will deny. The work that prevents that denial happens in November and December: fee schedules, EHR charge masters, authorizations and payer contracts all need the new codes before the first visit of the year.

What did the AMA release for CPT 2027?

The American Medical Association released the CPT 2027 code set on September 9, 2026. The biggest areas are maternity care, ABA, sleep testing and biofeedback. The release also adds new codes for AI services.

The table below covers the changes that reach small and specialty practices.

Specialty What changes on January 1, 2027 Billing risk
ABA Six new codes, all of 97151 to 97158 revised, 0362T and 0373T deleted Authorizations written on deleted codes
OB/GYN and family medicine with OB 17 global maternity codes deleted, 12 new codes added Global package billing stops working
Pediatric and adult speech therapy 92507 deleted and replaced by a new series Every SLP treatment claim
Pelvic floor PT, behavioral health Biofeedback revised to a time based structure Units and documentation of time
Primary care E/M No changes to 99202 to 99215 or telehealth codes 98000 to 98016 Low
Psychotherapy No CPT 2027 changes to 908xx codes Low
Pediatric vaccines No changes to 90460 and 90461 Low

What changes for ABA billing in 2027?

ABA has the heaviest rewrite of any outpatient specialty this year. The CPT Editorial Panel added six new codes and revised every code from 97151 to 97158. It also deleted 0362T and 0373T. Practices use those two codes today for severe destructive behavior.

According to the ABA Coding Coalition, the new codes cover:

  1. Assessment of harmful behavior with two technicians, plus an add on code for each additional technician
  2. Treatment of harmful behavior with two technicians, plus an add on code
  3. Face to face treatment with analysis delivered by the qualified health care professional
  4. Work the qualified health care professional does away from the client, such as data review, plan changes and technician training

That last item matters to every BCBA. Time spent on data review and treatment plan changes outside the session becomes billable for the first time under a dedicated code.

The risk sits in the authorizations. Most ABA authorizations for 2027 were written in the fall of 2026, on 2026 codes. A claim for a deleted code denies outright. A claim for a new code with no matching authorization denies for missing authorization. Payers will also adopt the new codes on different dates, and Medicaid programs often lag commercial plans.

Steps for ABA practices before January 1:

  1. List every active authorization that references 0362T, 0373T or 97155.
  2. Ask each payer, in writing, for its crosswalk and its effective date for the new codes.
  3. Update the EHR, the fee schedule and the session note templates so the billed code matches the documented service.
  4. Hold January claims for one check against the payer crosswalk before submission.

The CY2027 Medicare proposed rule keeps codes 97151 to 97158 on the Medicare telehealth list. Whether the six new codes join that list is not yet decided. For the denial patterns already showing up in ABA this year, see our breakdown of ABA billing denials in 2026.

What changes for maternity billing in 2027?

The global obstetric package is gone. CPT 2027 deletes 17 global maternity codes, including 59400, 59510, 59610 and 59618. It adds 12 new codes: 59080 to 59083, 59431 to 59434, 59502 to 59504 and 59623.

Prenatal and postpartum visits are now billed one at a time as E/M services. They are no longer bundled into one charge at delivery. That changes three things for an OB or family medicine practice:

  1. Revenue now arrives across the pregnancy, in many smaller payments.
  2. Each prenatal visit needs a note that supports its own E/M level.
  3. Each visit is a separate claim, so eligibility and payer rules apply visit by visit.

Practices that still bill a global code on a 2027 date of service will receive a denial for an invalid code.

What changes for speech therapy in 2027?

Code 92507, the core speech language pathology treatment code, is deleted on January 1, 2027 and replaced by a new code series. Pediatric therapy groups, pediatric practices that employ SLPs and multi discipline therapy clinics all need the new codes loaded before January.

The CY2027 Medicare proposed rule also includes a new pediatric speech therapy G code. Its final number arrives with the final fee schedule rule, expected in early November.

What changes for biofeedback?

Biofeedback code 90901 is revised and a new code is added under a time based structure. Pelvic floor PT and behavioral health practices that bill biofeedback must record session time in the note. The time has to support the units billed.

Which ICD 10 changes are already in effect?

The FY2027 ICD 10 CM code set took effect on October 1, 2026, three months ahead of CPT. Two changes are already causing rejections in primary care and therapy practices:

  1. M72.2 (plantar fascial fibromatosis) is now a header and is no longer billable. Plantar fasciitis has new specific codes, M67.A01, M67.A02 and M67.A09.
  2. Z68.1 (BMI 19.9 or less, adult) is now a header and is replaced by Z68.18 and Z68.19.

Any claim dated October 1, 2026 or later that still carries either code will reject as a nonbillable diagnosis. Check your EHR favorites lists and problem lists, which often keep old codes long after the code set changes.

Does Medicare change anything else on January 1, 2027?

Yes, but most of it is still proposed. CMS released the CY2027 Physician Fee Schedule proposed rule on July 14, 2026, and the final rule is expected around November 1. The proposals small practices should watch:

  1. The conversion factor drops to $32.8409 for most clinicians, a cut of 1.68%, mainly because the one year 2.5% bump in 2026 expires.
  2. G2211 would be replaced by a payment modifier that adds 16% to the E/M visit.
  3. An E/M visit billed on the same day as a minor procedure with a global period would be paid at 50%. We covered this in detail in the 2027 Modifier 25 payment cut.
  4. Remote patient monitoring would be payable only when the monitoring staff are employed by the practice.

We will update this article when the final rule is published.

How does WeBill Health prepare client practices for CPT 2027?

WeBill Health maintains internal payer rule libraries organized by specialty and by carrier, across more than 500 payer rulesets. When a code set changes, an expert updates the library with each payer’s crosswalk and start date. Every claim then receives human pre submission review against it before it goes out. For CPT 2027, that review catches deleted codes before they reach the payer. It also matches each authorization to the code the payer will accept in January.

The same review stands behind WeBill Health’s 98% clean claim rate.
