Denial Management Companies Compared for 2026

Average initial denial rate of 11.81 percent in 2024

Companies that offer medical billing denial management solutions in the USA fall into four groups. Specialty focused billing partners such as WeBill Health serve independent practices. Large outsourced RCM firms such as R1 RCM, AGS Health and Access Healthcare serve health systems and physician groups. Revecore concentrates on hospital recovery. Software companies such as athenahealth, AdvancedMD and CareCloud build denial work into their billing products.

The right fit depends on practice size, coding complexity, and whether your biggest leak is preventable denials or unworked ones. Below, eight companies are compared using only what each one publishes about itself.

What denial management covers in 2026

Denial management has two halves. Prevention stops a claim from being denied in the first place, through eligibility checks, authorization tracking, coding review and payer rule checks before submission. Recovery works the claims that were denied anyway, through root cause analysis, corrected claims and appeals filed inside each payer’s deadline.

The recovery half is where most practices lose money without noticing. KFF’s March 2026 analysis of federal transparency data on HealthCare.gov Marketplace plans found that insurers denied 19% of in network claims in 2024, and that fewer than 1% of denied claims were ever appealed. Insurers upheld 66% of the denials that were appealed, so roughly one in three appealed denials did not stand. A denial that nobody appeals becomes a write off by default.

Prevention matters just as much. In the same KFF data, missing prior authorization or referral accounted for 9% of in network denials, and administrative reasons accounted for 25%. Both categories can be fixed before a claim leaves the office. Our breakdown of referral and authorization denials walks through the versions we see most often.

Denial management companies in the USA: eight options compared

Every company below was checked against its own website in October 2026. For each one we list who it says it serves and what denial work it names. We did not repeat performance figures a company has not published itself.

1. WeBill Health

WeBill Health is a specialty focused revenue cycle management and revenue defense partner headquartered in Mountlake Terrace, Washington. It works with independent practices in three specialty clusters. Office Visit and Consultation Billing covers family medicine, internal medicine, pediatrics, geriatrics, urgent care and psychiatry. Time Based Therapy Billing covers psychology, LCSW practices, family therapy, substance abuse counseling, ABA, neuropsychology, physical therapy, occupational therapy and chiropractic. Procedure and Surgical Billing covers physiatry and PM&R, orthopedics, pain management, podiatry, OBGYN and cardiology. Denial work runs through its Denial Defense 2.0 program, which pairs review before submission with appeals after a denial.

2. R1 RCM

R1 RCM sells revenue cycle solutions to providers “across the care continuum,” with a dedicated track for physician groups and critical access hospitals. Denials management is listed as a back end solution inside its Phare Flow product, next to A/R follow up and payment recovery.

3. AGS Health

AGS Health lists its US office in Washington, DC, with operations in Chennai, India, and describes more than a decade of industry experience. It serves hospitals, health systems and physicians, and its site also names urgent care, ambulatory surgery centers, physical therapy and rehabilitation centers, and behavioral health. Its A/R management menu includes denial management and prevention, clinical denials and appeals, and payment variance and underpayment services.

4. Access Healthcare

Access Healthcare lists a US office in the Dallas area of Texas and an operations center in Chennai, India. It describes its clients as health systems, physician practices and billing companies, and it also lists payers and dental practices. Denial management appears under its back end revenue cycle services, alongside remittance processing.

5. Revecore

Revecore is based in Franklin, Tennessee, and concentrates on revenue recovery for hospitals and health systems. It cites more than 1,300 hospital clients and 26 years of recovery work. Its services include clinician led denial resolution and recovery of claims that were underpaid after adjudication. That focus makes it a strong option for hospital underpayments and a weak match for a solo therapy or primary care practice.

6. athenahealth

athenahealth offers revenue cycle software and services through athenaOne for practices and athenaIDX for enterprise organizations. Its athenaOne page says pricing is based on a percentage of collections and that its billing experts resolve claim errors before submission. The athenaIDX page describes A/R follow up and denial pursuit at enterprise scale. Denial work comes bundled with the athenahealth EHR and practice management system, so it suits practices that are willing to change platforms.

7. AdvancedMD

AdvancedMD sells practice software and a managed billing service to independent practices and larger groups. Its billing service page says its team works unpaid claims, appeals, secondary filings, exclusions and denials, and that the service includes payment posting and A/R aging reports. Its FAQ states that typical RCM services are priced at about 4 to 8% of monthly collections, and AdvancedMD quotes its own rate on request.

8. CareCloud

CareCloud is a Nasdaq listed company offering practice management software and revenue cycle services to small practices, large medical groups, health systems and billing companies. Its site does not name a standalone denial management service. The closest feature is an appeals tool that drafts appeal letters for staff to review and send. Pricing is not published.

How to choose a denial management partner

Start with practice size

Enterprise firms build their teams around hospital claim volume, contract underpayments and large denial backlogs. A practice with one to five providers has a different problem. It needs someone who knows its payer mix by name and fixes the cause of a denial before the next batch goes out. Ask each company what share of its clients look like your practice.

Check specialty depth before anything else

Denial causes change by specialty. Time based therapy codes fail on unit counts and session documentation. Office visit practices see modifier 25 and E/M level disputes. Procedure practices lose claims to missing authorizations and bundling edits. Payer behavior shifts too, as our state by state review of BCBS downcoding in 2026 shows. Ask for the coders who would work your claims and how many practices in your specialty they currently support.

Ask how much of the work happens before submission

Appeals recover money slowly and cost staff time on both sides. A partner that catches eligibility gaps, authorization gaps and coding conflicts before the claim leaves will generate fewer denials to appeal in the first place. Ask what is checked on every claim, who checks it, and against what payer rules.

Insist on root cause reporting

A monthly report that lists denied dollars tells you very little. You need denials grouped by reason code, payer and provider, with the fix that was applied and whether the same denial came back.

Compare pricing on the same claims

Pricing models include a percentage of collections, a flat monthly fee, a per claim fee and contingency fees on recovered dollars. Run each quote against your last three months of collections and denials so the numbers are comparable. Our guide to how much medical billing companies charge explains each model and where hidden costs tend to sit.

Questions to ask any denial management company

  • Which specialties make up most of your current client base?
  • Who reviews a claim before it is submitted, and what rules do they review it against?
  • How do you track appeal deadlines for each payer?
  • Can I see a sample root cause report with patient data removed?
  • What happens to my data and open A/R if I end the contract?
  • How will I reach the people working my claims when a question comes up?

How WeBill Health defends against denials

WeBill Health maintains internal payer rule libraries organized by specialty and by carrier, built and updated by experts who work claims in your specialty every day. Every claim receives human pre submission review against them before it goes out. Those libraries cover more than 500 payer rulesets. That review sits behind the 98% clean claim rate and 48 hour submission guarantee.

Prevention starts upstream. Real time eligibility verification confirms coverage before the visit, and authorization and referral requirements are checked against the payer’s current rule for that service. When a denial does arrive, the team traces it to its root cause, corrects the claim or files the appeal inside the payer’s window, and updates the rule library so the same denial is eliminated at the source for every future claim.

The financial result shows up in A/R. WeBill Health practices see a 40% A/R reduction within 90 days, with real time A/R aging visibility so the owner can see what is outstanding and why. Practice owners get direct access to their billing team through tools for seamless communication. Denial defense sits inside a full end to end revenue cycle management service, so the same people who prevent denials also post payments, follow up on A/R and keep credentialing current.

Frequently asked questions

What is a denial management company?

A denial management company prevents and recovers denied insurance claims for healthcare providers. The work includes reviewing claims before submission, finding the root cause of each denial, correcting and resubmitting claims, and filing appeals before payer deadlines expire.

Which revenue cycle management consultants specialize in denials reduction?

AGS Health, Access Healthcare and R1 RCM offer denial services mainly to health systems and large groups, and Revecore focuses on hospital recovery. For independent practices, WeBill Health specializes in denial prevention by specialty, using payer rule libraries and human review before every submission.

Is outsourced denial management worth it for a small practice?

It is worth it when denials are going unworked or the same denial reasons keep repeating. KFF found that fewer than 1% of denied Marketplace claims were appealed in 2024, which shows how much revenue is simply left behind when nobody follows up.

How do denial management companies charge?

Most charge a percentage of collections, a flat monthly fee, a per claim fee, or a contingency fee on recovered dollars. AdvancedMD’s FAQ cites a typical range of 4 to 8% of monthly collections for RCM services. Compare quotes against your own claim history.

How fast should a denied claim be appealed?

As soon as the root cause is confirmed, and always inside the payer’s deadline. For Medicare Part A and Part B, CMS allows 120 days from receipt of the initial determination to request a redetermination. Commercial payer deadlines vary by contract and are often shorter.

Find out where your denials are coming from

If you are comparing denial management companies, start with your own numbers. A Revenue Defense Audit reviews your recent denials by payer, reason code and provider and shows which ones could have been stopped before submission. If you want a faster read first, the Revenue Defense Score takes a few minutes and grades how exposed your practice is to the denial patterns covered above.
