The best revenue cycle management (RCM) services in the USA fall into three groups. Enterprise companies such as R1, Optum and Conifer Health run revenue cycles for hospitals, health systems and large physician groups. Software platforms such as Waystar and athenahealth give a practice’s own staff the tools to run billing. Specialty partners such as WeBill Health run the whole revenue cycle for independent practices. The right choice depends on your size, your specialty and whether you have billing staff of your own.

This guide explains what full RCM should cover, compares the three groups, and gives you the benchmarks and questions to hold any company to before you sign.

## What does a revenue cycle management service cover?

Revenue cycle management covers every step between scheduling a patient and collecting the last dollar for that visit. Medical billing is one part of it. A full RCM service covers three stages.

| Stage | What it includes | What goes wrong when it is missed |
|---|---|---|
| Front end | Eligibility verification, prior authorization, registration, provider credentialing | Denials for coverage, authorization and enrollment |
| Middle | Coding, charge capture, claim review and submission | Coding denials, downcoding and late filing |
| Back end | Payment posting, denial follow up, appeals, patient statements, A/R recovery | Unworked denials and aging A/R |

Most denials start at the front end. In Experian Health’s 2025 survey, providers named missing or inaccurate data (50%), authorizations (35%) and registration errors (32%) as their top denial causes. An RCM service that only touches the middle stage leaves those causes in place. ## The three kinds of RCM companies

Each company below is described from what it states on its own website as of October 2026.

### Enterprise RCM companies: R1, Optum and Conifer Health

R1 describes a revenue operating system that combines AI with human expertise. It says it partners with 95 of the top 100 US health systems and also offers solutions for physician groups and critical access hospitals.

Optum offers ambulatory RCM services that combine people, processes and technology, including denial reduction, appeals and EHR integration. Its page names anesthesia, emergency medicine, pathology, radiology and telehealth among the specialties it serves.

Conifer Health offers revenue cycle and value based care services to hospitals, health systems, physician groups, and employers and unions.

These companies are built for high volume and large organizations. A three provider clinic can buy from them, but it will be a small account inside a very large book of business.

### RCM software platforms: Waystar and athenahealth

Waystar sells cloud based software for end to end revenue cycle management. Its customers include physician and specialty practices, health systems and hospitals, and billing companies that serve them. Your own billing staff use it to run the work.

athenahealth combines an EHR with revenue cycle tools and services. Its enterprise RCM offering targets large and multisite organizations, while smaller practices are pointed to its athenaOne product.

A platform fits a practice that already has experienced billing staff and wants better tools for them.

### Specialty RCM partners: WeBill Health

WeBill Health runs the full revenue cycle for independent practices, from eligibility and credentialing through coding, submission, denial defense and A/R recovery. It covers 21 specialties in three groups: Office Visit and Consultation, Time Based Therapy, and Procedure and Surgical. Its fee is a share of what it collects, so WeBill Health earns more only when the practice collects more.

A specialty partner fits a practice with 1 to 15 providers that has no billing department of its own and wants one team responsible for the result.

| Type | Examples | Who runs your billing | Best fit |
|---|---|---|---|
| Enterprise RCM | R1, Optum, Conifer Health | The company’s large operations teams | Health systems, hospitals, large physician groups |
| Software platform | Waystar, athenahealth | Your own staff, using the platform | Practices with experienced billing staff |
| Specialty partner | WeBill Health | A team assigned to your specialty | Independent practices without a billing department |

## What benchmarks should an RCM service hit?

Ask every company for its numbers on these measures, and compare them with industry benchmarks. These figures come from MGMA and the MGMA and Conifer physician RCM KPI framework, summarized in our [2026 denial benchmarks white paper](https://webillhealth.com/2026-denial-benchmarks/).

| Measure | Industry benchmark |
|---|---|
| Initial denial rate | 5% to 10% typical, under 5% best practice |
| Clean claim rate | 98% |
| Days in accounts receivable | 30 to 40 |
| A/R older than 90 days | Under 10% |
| Net collection rate | 95% minimum, 97% to 99% optimal |

Ask how each number is measured. A denial rate that leaves out clearinghouse rejections or payer takebacks will look better than it is. Kodiak Solutions found that takebacks after payment rose to 1.57% of net revenue in the first half of 2026, so a falling denial rate alone does not prove that collections improved.

## How do RCM companies charge?

Most RCM companies charge a percentage of collections. Some charge a flat fee per claim or per provider, and some combine a small base fee with a lower percentage. AdvancedMD, for example, states that typical RCM services are priced at about 4 to 8% of monthly collections.

A percentage model ties the company’s income to what you collect. Whatever the model, ask for a written list of what the fee includes and what costs extra, such as credentialing, patient statements or work on old A/R. ## What changes RCM has to handle in 2027

Any RCM company you hire now has to be ready for January 1, 2027.

1. CPT 2027 brings 453 code changes, including a rewrite of ABA codes, the deletion of speech therapy code 92507 and the end of global maternity billing. See [CPT 2027 changes for small practices](https://webillhealth.com/cpt-2027-code-changes-small-practices/).
2. Medicare Advantage, Medicaid and marketplace plans must run electronic prior authorization APIs. See [prior authorization changes in 2027](https://webillhealth.com/prior-authorization-changes-2027/).
3. CMS has proposed a 1.68% cut to the Medicare conversion factor for most clinicians. See [what the CY2027 fee schedule means for a 3 to 5 provider practice](https://webillhealth.com/what-the-cy2027-fee-schedule-cut-means-for-a-3-to-5-provider-practice/).
4. Medicare Advantage plan exits move patients to new plans, which may require new provider enrollments. See [the best credentialing and enrollment companies for 2027](https://webillhealth.com/which-companies-offer-fully-managed-credentialing-and-enrollment-services/).

## Questions to ask before you sign

Ask each company the same questions and get the answers in writing.

1. What are your denial rate, clean claim rate and days in A/R for practices in my specialty and size, and how do you measure each one?
2. Who checks a claim before it goes out, and do they know my specialty’s codes and payers?
3. What share of denials do you appeal, and how fast?
4. Do you handle eligibility, prior authorization and credentialing, or only claims?
5. What reports will I get, how often, and who do I call when something looks wrong?
6. What does your fee include, and what costs extra?
7. Can you give me two references from practices like mine?

A company that cannot answer the first question with numbers is unlikely to manage your revenue by them.

## How WeBill Health runs the revenue cycle

WeBill Health maintains internal payer rule libraries organized by specialty and by carrier, built and updated by experts who work claims in your specialty every day. Every claim receives human pre submission review against them before it goes out. There is no black box and no algorithm we ask you to take on faith. The libraries cover more than 500 payer rulesets.

That review stands behind a 98% clean claim rate and a 48 hour submission guarantee, with 7 to 14 day reimbursement velocity. Clients see a 40% A/R reduction within 90 days, and practices average a 25% increase in collections within six months, driven by specialty specific coding and fewer days in A/R.

Clinical Virtual Assistants handle eligibility and prior authorization before the visit. New providers enroll through Revenue Velocity Credentialing, with 40% faster credentialing enrollment and 24/7 CAQH and PECOS lifecycle monitoring. Practices get weekly velocity reports, A/R aging they can see at any time, and tools for seamless communication with the team that works their claims. ## Frequently asked questions

### What is the best RCM company in the USA?

It depends on the organization. Health systems and hospitals usually fit enterprise companies such as R1, Optum or Conifer Health. Practices with their own billing staff may prefer software such as Waystar or athenahealth. Independent practices without a billing department fit a specialty partner such as WeBill Health, which runs the full revenue cycle with a 98% clean claim rate.

### What is the difference between RCM and medical billing?

Medical billing covers coding and submitting claims and following up on payment. Revenue cycle management covers the whole process, including eligibility checks, prior authorization, credentialing, denial prevention, appeals and A/R recovery.

### How much do RCM services cost?

Most RCM companies charge a percentage of collections. AdvancedMD states that typical RCM services are priced at about 4 to 8% of monthly collections. The rate depends on specialty, claim volume and the services included.

### What is a good denial rate for a medical practice?

MGMA data puts the typical first submission denial rate for medical practices at 7% to 8%, and well run practices can get under 5%.

### Is outsourcing RCM worth it for a small practice?

It is usually worth it when denials, slow payments or credentialing gaps are already costing more than the fee. See [the real benefits of outsourcing medical billing for independent practices](https://webillhealth.com/the-real-benefits-of-outsourcing-medical-billing-for-independent-practices/).
